Your first visit
Call 952-831-8742 to arrange a consultation. When you come for your appointment, please:
- Arrive 10–15 minutes early to register.
- Bring your driver’s license or a valid ID, and your insurance information.
- Bring any relevant reports, X-rays, MRI scans, or CT scans.
- Bring a current list of your medications, including supplements.
If you need to cancel
Please telephone the office during business hours and allow at least one day’s notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
